Video-Assisted Ablation of Pilonidal Sinus
The video-assisted ablation of the pilonidal sinus (VAAPS) is a new 5-phase treatment option available to remove pilonidal cysts. It boasts endoscopic minimally invasive treatment that rivals the standard sinusectomy for pilonidal sinus disease.
The procedure starts with inserting the endoscope through the lower external pit (orifice) for identifying the lateral tracks of the sinus cavity using saline solutions. The third phase identifies the presence of hair and removes it before obtaining complete ablation of the sinus cavity using electrodes maneuvered with a continuing, direct vision for adequately cleaning the sinus cavity. By letting the saline solution to flow through, the procedure eliminates all traces of necrotic material. With the use of a Volkmann spoon the sinus cavity is cleansed while maintaining minimal surgical wounds.
Although more studies are needed to prove that VAAPS can effectively treat pilonidal sinus, its minimally invasive nature and better postoperative requirements are attractive in treating pilonidal cysts.
